We are an experienced health care facility that offers post-acute health care services designed to assist patients in rebuilding strength, functioning and independence following a hospital stay for an acute illness or injury.

It is our goal to return our patients to the highest levels of independence and we support our residents in returning home whenever possible.

The Lincoln Centers provide outstanding long-term eldercare services to those residents who need assistance in their daily activities of living and for whom living independently at home is no longer possible.
